Your Medical Record

The Health Insurance Portability and Accountability Act ( HIPAA) guarantees that all medical records are kept confidential. Parkland Medical Center is in full compliance with this regulation.

To receive a copy of your medical record we require a written request. Click here to download our authorization form.

Our office hours are Monday thru Friday 8:00 a.m to 4:30 p.m.

Records being released to another medical office for continuation of care are sent at no cost to the patient. Personal requests for a complete copy of your medical record are charged a base fee of $15.00 for the first 30 pages and $.50 for each additional page. Personal requests for an abstract (includes pertinent dictated reports and tests) of your medical record is free of charge for the first copy.  Please allow 5 to 7 business days to complete your request. You will recieve a phone call when your records are ready to be picked up. Please be prepared to show your photo id when picking up your records.
